Hansen’s disease presenting as sporotrichoid subcutaneous cysts

Hansen’s disease (HD) is known for its varied clinical presentations. We report a case of a 28-year-old male presenting with multiple asymptomatic subcutaneous cysts in a sporotrichoid pattern on his left lower limb. The patient presented without obvious neurological deficits, which misled the diagnosis. Relevant investigations and non-response to empirical therapy helped to rule out various differential diagnoses, including atypical mycobacterial infections, sporotrichosis and phaeohyphomycosis. The diagnosis of Hansen’s disease was achieved through MRI and highresolution ultrasonography (HR USG) by demonstrating nerve thickening and nerve abscesses along the common peroneal and superficial peroneal nerve respectively, and nerve conduction studies, which revealed absent sensory neural action potentials in the superficial peroneal nerve and reduced amplitude with prolonged latency of the common peroneal nerve’s motor action potential.
