[Unilateral lachrymal gland enlargement revealing sarcoidosis in an 85-year-old man].
Abstract
We present a case of an 85-year-old Caribbean man initially presenting with lachrymal gland enlargement and a large subcutaneous extension responsible for a hemifacial "lion-like" deformation. The most important diagnoses to suggest are adenocarcinoma of the lachrymal gland, lymphoma, leprosy, tuberculosis, and sarcoidosis. Based on the clinical, biological, and radiological findings, sarcoidosis was suspected with lachrymal gland and pulmonary lesions. Biopsy of the enlarged lachrymal gland revealed a non caseating granuloma compatible with the diagnosis of sarcoidosis. The value of this case is the atypical field and sarcoidosis revealed as a unilateral lesion in an elderly male.

Nous rapportons le cas d’un homme antillais, âgé de 85 ans, ayant présenté une hypertrophie de la glande lacrymale droite avec extension sous-cutanée majeure responsable d’une déformation hémifaciale de type léonien. Les principaux diagnostics évoqués étaient l’adénocarcinome de la glande lacrymale, le lymphome, la lèpre, la tuberculose et la sarcoïdose. Les bilans cliniques, biologiques et radiologiques ont permis de poser le diagnostic de sarcoïdose systémique avec atteinte orbitaire et pulmonaire. Le diagnostic a été confirmé par l’analyse histopathologique d’une biopsie de la glande lacrymale. Dans ce cas, le terrain et le mode de révélation de la sarcoïdose étaient atypiques : lésion unilatérale, patient âgé, de sexe masculin.
