TIENE LEPRA ESTE PACIENTE?

Presentamos el caso de un paciente de 28 años de edad con manchas y parches hipocrómicos del tronco, sin alteración de la sensibilidad. El paciente deseaba saber si tenía lepra pues dos hermanos y su abuela la padecían. La baciloscopia fue negativa y la biopsia demostró hifas y esporos en la capa córnea. Los factores de riesgo y la clínica del paciente indicaban inicialmente sospecha de enfermedad de Hansen, sin embargo, el reporte histopatológico permitió concluir que el paciente cursaba con pitiriasis versicolor, un diagnóstico diferencial clínico importante de la lepra, principalmente en su fase inicial.

We present the case of a 28-year-old patient with hypochromic spots and patches on the trunk, without altered sensitivity. The patient wanted to know if he had leprosy because two brothers and his grandmother suffered from it. The smear microscopy was negative and the biopsy showed hyphae and spores in the corneal layer. The risk factors and the patient’s symptoms initially indicated suspicion of Hansen’s disease; however, the histopathological report allowed us to conclude that the patient had pityriasis versicolor, an important clinical differential diagnosis of leprosy, mainly in its initial phase.
