Intervenção Da Terapia Ocupacional Associada Ao Protocolo Atividade Exercício (Pae): Um Relato De Experiência Com Pacientes Com Sequelas Neurológicas De Hanseníase

O presente estudo objetiva descrever a efetividade terapêutica do PAE, sobre o Componente de Desempenho Força Muscular (FM), em um paciente hansênico, com incapacidade, decorrente de dano neural nas mãos. Trata-se de uma pesquisa do tipo relato de experiência decorrente da atividade curricular, na Prática Clínica, do curso de Terapia Ocupacional, da Universidade do Estado do Pará (UEPA). Os atendimentos ocorreram na Unidade de Ensino e Assistência de Fisioterapia e Terapia Ocupacional (UEAFTO), no período de dois meses (outubro e novembro de 2021). Foi aplicado o PAE para a reabilitação da mão direita (MD) e da mão esquerda (ME). Para os membros inferiores (MMII), utilizou-se recursos terapêuticos como Thera Brand, circuitos psicomotores e alongamentos, preconizados pelo manual de prevenção de incapacidade do Ministério da Saúde. Na avaliação com o PAE o paciente apresentou sequelas hansênicas (garra ulnar) na MD, déficit da FM na MD e na ME, sequela neurológica no membro inferior esquerdo (MIE) e déficit funcional no membro inferior direito (MID). Com aplicação do PAE e demais abordagens terapêuticas, como alongamentos corporais e atividades psicomotoras, observou-se melhora na adesão ao tratamento. Identificou-se a preservação funcional da MD e ME, diminuição do avanço das deformidades da MD e da amplitude de movimento (ADM) da MD e da ME, assim como melhora funcional dos MMII. A partir do uso do PAE, identificou-se a necessidade de confecção de uma órtese para garra ulnar, objetivando modular e estimular o membro acometido, sendo este o MD. Identificou-se ganhos terapêuticos sobre o Componente de Desempenho FM, na MD e ME, facilitando o desempenho em atividades de vida diária (AVDs), gerando bem-estar e mais qualidade de vida, conforme relato do paciente. Diante dos resultados apresentados, a intervenção terapêutica ocupacional, associada ao uso do Protocolo de Atividade Exercício (PAE), demostra-se efetiva no tratamento de paciente com sequela neurológica de hanseníase.

English Abstract:

The present study aims to describe the therapeutic effectiveness of PAE, on the Performance Component Muscular Strength (FM), in a leprosy patient, with disability, resulting from nerve damage in the hands. This is an experience report type research resulting from the curricular activity, in Clinical Practice, of the Occupational Therapy course, at the State University of Pará (UEPA). The consultations took place at the Physical Therapy and Occupational Therapy Teaching and Assistance Unit (UEAFTO), in the period of two months (October and November 2021). The PAE was applied for the rehabilitation of the right hand (RM) and the left hand (LM). For the lower limbs (LLLL), therapeutic resources such as Thera Brand, psychomotor circuits and stretching, recommended by the Ministry of Health's disability prevention manual, were used. In the evaluation with the EAP, the patient presented leprosy sequelae (ulnar claw) in the MD, FM deficit in the MD and EM, neurological sequelae in the left lower limb (LLL) and functional deficit in the right lower limb (RLL). With the application of PAE and other therapeutic approaches, such as body stretching and psychomotor activities, an improvement in adherence to treatment was observed. Functional preservation of MD and EM, decrease in the progression of MD deformities and range of motion (ROM) of MD and ME, as well as functional improvement of lower limbs were identified. From the use of the EAP, the need to manufacture an orthosis for the ulnar claw was identified, aiming to modulate and stimulate the affected limb, which is the MD. Therapeutic gains were identified on the FM Performance Component, in DM and EM, facilitating performance in activities of daily living (ADLs), generating well-being and better quality of life, according to the patient's report. In view of the results presented, the occupational therapeutic intervention, associated with the use of the Exercise Activity Protocol (EAP), proves to be effective in the treatment of patients with neurological sequelae of leprosy
