Objective: To characterize the clinical-epidemiological profile of leprosy cases in the city of Coroatá, Maranhão.

Methods: Epidemiological, retrospective, descriptive and quantitative study. Data from patients with leprosy were used, referring to the period from 2012 to 2021, obtained from the database of the Notifiable Diseases Information System, made available by the Department of Informatics of the Unified Health System.

Results: In the analyzed period, 646 cases of leprosy were reported. The frequency was higher in males (56.7%) and in the 15-59 age group (60.1%). As for clinical aspects, 56.3% were borderline, 74.9% classified as multibacillary and 60.1% without physical disabilities. 226 cases (34.9%) presented some degree of disability. The diagnosis of leprosy is still carried out late due to the detection of the multibacillary classification and borderline clinical form, in addition to the presence of physical disability at the time of diagnosis.

Conclusion: The results showed that leprosy remains a serious public health problem in the municipality, requiring the implementation of public policies that directly impact the indicators for reducing the disease
