AB - INTRODUCTION AND OBJECTIVES: Leprosy is an infectious disease that causes profound changes in multiple spheres related to an individual's health. It is a major public health issue in developing countries like Brazil. It is treated according to its clinical presentation, with multiple antibiotic regimens sometimes used in its management. One of these antibiotics is dapsone, a widely used medicine that, among its side effects, causes aplastic anemia, also knows as bone marrow aplasia, a general malfunction in the function of the bone marrow to produce blood cells. The purpose of this report is to provide a fatal case of bone marrow aplasia related to the use of dapsone. CASE REPORT: A 70-year-old male patient with prostatic adenocarcinoma and subsequently diagnosed as having leprosy. Treated with antibiotic regimen containing dapsone. It evolves, in the sixth dose, with pancytopenia (Erythrocytes: 1.2 million / mm³; Leukocytes: 1.800 million / mm³; Platelets: 214 billion / mm³) and is diagnosed as having bone marrow aplasia. The medication is promptly suspended, the patient gets worse, so he is hospitalized and then dies. CONCLUSIONS: Dapsone is an important drug in the management of leprosy and other diseases. The pathogenesis underlying the aplasia caused by this drug is still unknown. Constant assessment of the hematological patterns of patients using this drug is imperative. Research is also needed to clarify the possibility that the management of adenocarcinoma has played a role in the emergence of bone marrow aplasia in this patient.
