AB - <p>A case of lepromatous leprosy with erythema nodosum leprosum (ENL) undergoing treatment with dapsone, rifampin, and thalidomide developed focal tuberculoid granulomas in the ENL lesions. This is the first report known to the authors of a lepromatous leprosy patient in whom ENL and an upgrading reaction occurred simultaneously.</p>
