The disease affects people of any sex or age group, advancing slowly and progressively and, when untreated, can cause deformities and often irreversible physical disabilities. The study outlines the cases of leprosy underreporting investigated in 2018, in the state of Tocantins, considering clinical form and classification. A cross-sectional quantitative epidemiological study, using the database of the National System of Epidemiological Surveillance of the Ministry of Health and the SUS Department of Informatics (DATASUS), selected the state of Tocantins in 2018. The variables surveyed were: clinical form and classification. During the year of 2018, a total of 5,292 patients with leprosy were recorded, with cases under-reported being 42.25%, unspecified cases are excluded. The dysmorph clinical form presents higher rate of ignored cases (50.2%), followed by Indeterminate with 19.7%. The clinical forms, Virchowian and Tuberculoid, had respectively 18.3% and 11.6% ignored cases of the disease. Among the underreported cases, those with nerve damage, the forms were: 96% paucibacillary (less than or equal to 5 affected nerves) and 3.9% multibacillary (more than 5 affected nerves). Although it is a compulsory notification disease, it is a public health problem in Brazil. The diagnosis is essentially made in the Basic Health Services, so it is important for the health team to broaden the scope of enlightening actions, pointing to the prevention and clinical signs of leprosy in order to obtain an early diagnosis and better prognosis. a9786557062883
