Ungual changes are common in hanseniasis and attributed to a combination of factors such as neuropathy, trauma, vascular disorders and infections. It is estimated that 56% of paucibacillary patients and 87% of multibacillary patients present nail changes, of which the most common are: nail hyperkeratosis, onychorrhysis, onicorrhexis, micronycle, anonychia, Beau's grooves, scleronychia, longitudinal melanonychia and inverse nail pterygium. The present report presents ungual changes of leprosy in its rarest manifestation, of involvement of 20 nails and make an differential diagnosis with nail lichen planus, mucocutaneous candidiasis, pachydermoperiostosis, and other peripheral neuropathies such as Diabetes Mellitus.
